Background: Chronic diseases are the leading cause of global morbidity and mortality, with escalating healthcare costs and social burdens. Traditional models based on episodic, in-person care are ill-suited to manage the complex, long-term needs of these patients. Methods and Results: This review outlines the paradigm shift from conventional disease management toward integrated, technology-driven strategies. We highlight innovations such as telemedicine, digital therapeutics, artificial intelligence (AI)-based decision support, and personalized care pathways. Evidence from clinical trials and real-world studies is synthesized to demonstrate the impact of these tools on outcomes, adherence, and healthcare utilization. Conclusion: Innovative models centered on digital integration, predictive analytics, and patient empowerment offer new possibilities in chronic disease management. However, challenges in data privacy, access equity, and clinical validation remain barriers to full-scale implementation. |
